Description
The Neurodiverse Safe Work Initiative offers this NDSW Professional Coaching Program to assist you in mapping your own unique neurodiverse profile, understand how this impacts the way think and work and to develop skills to optimize work performance, safety, and health at work.
It is agreed that:
- All information provided by, and conversations shared between the parties during the term of this Agreement and thereafter will be confidential unless or until the Professional Coach is required by relevant legal authority to disclose them.
- All correspondence, reports, information, video and/or audio recordings generated during the term of the Professional Coaching Program will be kept in a secure location and facility. No person, except myself will have access to your file. The Professional Coach will only use such information to prepare for coaching sessions.
- For the purposes of obtaining and retaining Professional Coach Certification, the Client agrees for the Professional Coach to record the Client’s name, contact details, number, and dates of coaching sessions on a register for submission to the International Coaching Federation (ICF).
- Variations to this Agreement may be made at any time by the mutual consent of both parties.
- This Agreement can be cancelled by either party giving one month’s notice in advance. All monies due and payable up to this date must be paid in full.
- Session dates and times can be changed up to 24-hours in advance by either party to another mutually agreed time/date and this will incur no additional cost. However, sessions that are cancelled by the client with less than 24-hours’ notice will be charged at the usual rate.
- Professional Coaching Packages include the Client’s single use access to the Neurodiversity Workplace+ Profiler and Report and is valid for use withing 12-months from the date of this contract. Terms and conditions apply.
- The Neurodiversity Workplace Profiler is not a clinical diagnostic tool. The report will describe neurodivergent traits suggestive of Autism, ADHD, Dyslexia etc. These are not to be taken as evidence of a diagnosable medical condition.
- The Client owns their Neurodiversity Workplace Profiler Report which will be provided in .PDF format when the information provided by the Client has been analysed. The Client may use their report for any purpose consistent with the Terms and Conditions.
- If either party is not entirely happy with the coaching sessions or outcomes, we agree to discuss this immediately so that the situation can be resolved swiftly and amicably. At times a third party may need to be involved.
Payment for your chosen Coaching Package confirms your acceptance of the terms of this agreement.
